Procedure

​The SSW:

  1. Ensures that a DPP-111A Foster  Home Contract Supplement is provided to the foster parent(s) upon the foster home’s acceptance of a child;
  2. Ensures that the DPP-111A is signed within three (3) working days of placement in an emergency situation;
  3. Is to in​form the foster parent of any history of inappropriate sexual acts or other behaviors of the child that indicates a safety risk for placement, as with any other type of substitute care placement;
  4. Is mandated to inform the foster parent as soon as practical, but no later than seventy-two (72) hours after receiving the information, if information concerning inappropriate behaviors is not known at the time of placement;
  5. Files the original DPP-111A in the foster  home’s case file and within seventy-two (72) hours provides a copy of the signed DPP-111A Foster Home Contract Supplement to the:
    1. Foster home;
    2. Appropriate Billing Specialist; and
    3. Children's Benefits Worker (CBW);
  6. Completes a new DPP-111A Foster Home Contract Supplement if:
    1. A child's placement changes;
    2. A child's age qualifies him/her for a higher per diem rate; or
    3. The placement type is reclassified.
