3/15/23 Addition: Recruitment and Certification (R&C) staff will email the DPP-157 to Records Management for the adolescent age children.
Once enrolled in Kentucky Applicant Registry and Employment (KARES), an approved foster or adoptive parent and each adult member of the household shall maintain enrollment in KARES.
The rap sheet should never leave the KARES system.
2. Requests a copy of the applicants driver’s license or other government issued phone identification for verification that the photograph and name clearly match the individual submitting to the check.
3. Once the DPP 162 Applicant Waiver, DPP 163 Disclosure Statement along with a copy of the government issues photo identification have been received the worker will complete the KARES application process for the applicant and/or adult household member in order to obtain a child/neglect central registry check, sex offender registry address check, in state criminal records check conducted by Administrative Office of the Courts (AOC) and fingerprint scan criminal records of the National Crime Information Database (NCID).
4. Requests the applicant to provide the completed DPP 157 Background Check Request for Foster or Adoptive Applicants and Adolescent or Adult Household Members for all adolescent children between the ages of twelve (12) and seventeen (17) who resides in the home
7. Sends the completed Adam Walsh requirement forms to DAFM Records Management Section through the KARES system.
8. Uploads the KY KARES Final Results, AOC results and the completed DPP 157 for dependent children into TWIST.
3/15/23 Delete: The regionally trained staff conducts the fingerprint scan and faxes the DPP-157 to the Records Management Section at (502)564-9554.
1. Provides to the applicant or adult household member the DPP-162 Applicant Waiver Agreement and Statement and the DPP-163 Disclosure Statement to be signed by the applicant and adult household members. DPP-157 Background Checks for Applicants or Foster/Adoptive Parents to obtain a child abuse/neglect central registry check, sex offender registry address check or fingerprint scan criminal records of the National Crime Information Database (NCID) as required by SOP 12.4 Background Checks for Resource Parents and SOP 4.3 Relative and Absent Parent Search;
4. Requests that the applicant or adult household member provide the following to appropriate regional staff at one of the eighteen (18) regional fingerprint scan locations:
- The completed DPP-157 Background Checks for Applicants or Foster/Adoptive Parents; and
- Proof of identification (i.e., photo ID);
7. Provides the family with the appropriate forms from the Adam Walsh Requirements and Forms web page and requests that the family returns the completed forms. brings these forms along with the information listed in #2 to appropriate regional staff at one of the eighteen (18) regional fingerprint scan locations for their appointment.
