The SSW:
Considers if disability proceedings are indicated for one of the following reasons, through a review, at least six (6) months prior to the date of expiration of commitment of a youth committed to the cabinet who is physically or mentally incapable of managing the normal activities of daily living:
Guardianship; or
Extension of commitment;
Does not release commitment on individuals who appear to be incapable of making decisions or managing their own affairs until a plan has been developed to ensure their care.
Requests the involvement of staff from the Department for Aging and Independent Living, Division of Guardianship in planning for the youth and follows the procedures outlined in SOP 4.29.2 Transition Planning for OOHC Youth.
