C12.29 Aftercare Resources and Services

Introduction

​​​​​Some Chafee services and benefits for transition-age youth and young adults continue even after the young adult has exited out-of-home care (OOHC).​​

Practice Guidance


Services and supports for young adults who have exited care may include:
  • Educational support through tuition waivers or Education Training Vouchers;
  • Healthcare benefits;
  • Additional Aetna SKY benefits;
  • Aftercare services;
  • Earn and Learn;
  • Fostering Success;
  • Housing assistance through Project Life, FUP/FYI, or Mainstream;
  • Lifeset services;
  • RISE funds.




Procedure


The SSW:
  1. Supports youth when they exit OOHC at age eighteen (18) or older and ensures that they have all information and resources for a successful transition, following procedures in SOP C12.13 Exit from Care;
  2. Works in partnership with the transitional living specialist (TLS) to develop an aftercare plan with the young adult to supplement the transition plan, especially if additional resources or supports are needed that were not identified during transition planning, following procedures in SOP C4.3 CPS Aftercare Planning;
  3. Informs the TLS of the young adult’s plan to exit OOHC. 
The TLS:
  1. Encourages the young adult to create an ifoster account and save a copy of their vital documents to their digital locker; 
  2. Encourages the young adult to download the Kentucky RISE app to have quick access to information and resources;
  3. Upon being informed by the worker of a young adult’s intent to exit OOHC, provides information on community resources and supports, including those listed on the Kentucky Benefits Flyer for Aged Out Youth;
  4. Assists young adults who have exited OOHC with accessing resources upon request.

Other Transitional Services Branch Staff:
  1. Provide support and consultation to staff in the region, community partners, private child caring (PCC) and contract providers, and the TLS about supports and resources available to transition-age youth after they exit OOHC;
  2. Assess the needs of young adults who have exited OOHC as a whole and provide administrative support for obtaining services to meet those needs;
  3. Collaborate with the Ky RISE Voices of the Commonwealth (VOC) to improve services and support of young adults who have exited OOHC;
  4. Advocate for young adults who have exited OOHC through resource development, procurement, or development of training, and other administrative functions; and 
  5. Administer programs and services that support young adults who have exited OOHC, utilizing National Youth in Transition Database (NYTD) data and other data sources to assess needs and identify gaps in services and supports.
