23.2 Ongoing Services

Procedure

The SSW:
  1. ​​Provides ongoing services, including but not limited to: 
    1. Preventive services; 
    2. Counseling; 
    3. Transitional services; or 
    4. Referral or coordination of services, such as arranging transportation. ​
  2. Has personal contact with the adult at least every thirty (30) calendar days; 1 
  3. Identifies all ongoing services provided as tasks in the family level objective (FLO) or individual level objective (ILO); and 
  4. Documents all contacts with the client or on behalf of the client within five (5) working days of the contacts in the case file and in the appropriate TWIST screens, as outlined in SOP 1.4 Documentation;
​The FSOS: ​​​

  1. Conducts quarterly case reviews as detailed in SOP 1.5 Supervision and Consultation. ​
