27.5 Ongoing Services

Procedure

​The SSW:

  1. Once a general adult services case is opened provides ongoing services including, but not limited to:
    1. Preventive services;
    2. Counseling;
    3. Transitional services; or
    4. Referral or coordination of services, such as arranging transportation;
  2. Has personal contact with the adult each calendar month;  
  3. Identifies all ongoing services provided as tasks in the family level objective (FLO) or individual level objective (ILO); and
  4. Documents all contacts with the adult or on behalf of the adult within five (5) working days of the contact.


​Footnotes

  1. The expectation of such contacts is established in the case plan and agreed to by the adult or the adult's guardian.
