Trial Home Visitation or Extended Visitation
2. And family establishes guidelines for the trial home visits or extended visit, and a prevention plan is negotiated as outlined in SOP C4.2 CPS Prevention Planning, regarding the visitation and includes the following:
13. Ensures any extended visits last no longer than fourteen (14) calendar days if the plan is to return to the placement. H. A plan for early care and education services for children under age five (5) when needed to support the child’s ongoing development and stability in the home; and

4. For the safe infant pathway, status offender pathway, dependency pathway, and recommitment pathway, schedules trial home visits when the identified safety threats concerns in the home have been mitigated and the best interest of the child is reunification with the family, and should be:
5. For ongoing cases that, during the investigation, utilize the safety and risks assessments, the SDM™ Reunification Assessment will be completed when a child is being considered for a trial home visit.
D. Any therapeutic recommendations for the transition provided by community partners or the private child caring (PCC) or private child placing (PCP) agency, if applicable;
7. Follows TWIST procedures to ensure that the child’s placement is accurately captured in the system and use of the temporary living conditions (TLC) screens when appropriate, if the child is still committed, does not exit the child from out-of-home care (OOHC) even if they are at home;
