13.2 Child Freed for Adoption Procedure

Practice Guidance

  • For children who remain in out-of-home care (OOHC) with no identified adoptive home, the SSW uploads the child's updated presentation summary packet into the TWIST agency case upon approval of the packet.
  • A child-focused recruitment model (CFRM) specialist may also complete the presentation summary if the case is being worked in accordance with the CFRM.​

Procedure

The SSW:
  1. Creates a permanency/agency case for the child which is approved by the SSW’s FSOS; ​1
  2. Updates the DPP-AC-1-Transmittal Memorandum for Presentation Summary Packet and presentation summary in TWIST; 
  3. Ensures the presentation summary and all attachments are uploaded into the child’s agency case. This is completed in the case records/documents tab in TWIST by selecting the adoption section and the form description Presentation Summary and Attachments;
  4. Assess for referral to the CFRM specialist if the child(ren): 
    1. Is deemed eligible for special needs services, in accordance with 42 U.S.C. sec. 673; 
    2. Has a goal of planned permanent living arrangement (PPLA) or long-term foster care; 
    3. Is on extended commitment and parental rights have been terminated; or 
    4. Has adoption as the case plan goal and an adoptive resource has not been identified;
  5. Submits a Kentucky Adoption Profile Exchange (KAPE) referral to the Adoption Services Branch at the time of termination of parental rights (TPR) if an adoptive home has not been identified.3
  6. Refers a child to KAPE within thirty (30) calendar days if the child’s placement, which has previously been identified as an adoptive placement, disrupts or the adoptive parent(s) change their decision about adopting the child.​

Footnotes

  1. This should occur after the expiration of the thirty (30) calendar days period that the biological parents have to appeal the TPR finding after the judgement. If an appeal is filed, an agency case is not created until the appeal is resolved. 
  2. If the child has not previously been referred to CFRM,​ a dual referral to KAPE and CFRM should be made.

  • For children who remain in out-of-home care (OOHC) with no identified adoptive home, the SSW uploads the child's updated presentation summary packet into the TWIST agency case upon approval of the packet.  submits one (1) copy of the updated presentation summary to the central office Records Management Section within ten (10) days of the changes being made. 
3.  ​Ensures that the presentation summary and all attachments are uploaded into the child’s agency case. This is done in the case records/documents tab by selecting the adoption section and the form description presentation summary and attachments.Submits one (1) full copy with attachments to the central office Records Management Section; 2​ 

5.  Submits a Kentucky Adoption Profile Exchange (KAPE) referral to the Adoption Services Branch at the time of termination of parental rights (TPR) if an adoptive home has not been identified.3

2.  The recruitment and certification (R&C) worker submits the presentation summary packet to central office Records Management Section if the child’s foster parents adopted the child. The presentation summary packet and other required documents are submitted when the case is sealed. If a foster family is not adopting the child, the SSW submits the presentation summary packet to central office Records Management Section upon regional approval of the packet.​​​
